form should be filled out the child's pediatrician and
returned to the school nurse. You will need to print out and
complete one form for each child enrolled in the district.
Annual Medical History
Form: This form should be
filled out the child's parent or guardian and returned to the
school nurse. You will need to print out and complete one form
for each child enrolled in the district.
Annual Dental
Examination Form: This form should be filled out the child's dentist and returned to the school nurse. You
will need to print out and complete one form for each child
enrolled in the district.
Emergency
Contact Information:
Update your emergency contact information any time with this
form. It should be printed out and filled out by the parent or
guardian, and then returned to the school's main office.
