REGISTRATION UPDATE

Please be advised that if you are a new resident in the South Orangetown Central School District, and you would like your child to begin school on September 7, 2010, you must complete the registration packet and schedule an appointment with Rose Ngai on or before August 31, 2010. Call (845) 680-1137 for an appointment.


Welcome to the South Orangetown Central School District.  We hope that the following information will answer some of your questions.  Please do not hesitate to ask about special situations or problems.  Registration packets can be picked up at the district registrar's office located at the Middle School, 160 Van Wyck Road in Blauvelt.

Grade Placement
The Grade Placement assigned at registration is provisional until the principal or guidance counselor makes the appropriate final grade determination. 

Please note that your child may be required to take a mandatory screening exam.
The procedure may take up to one and a half hours and must be completed prior to meeting with a counselor for a schedule of classes.

If your child receives special services, please bring his/her latest Individual Educational Plan (I.E.P.)


To Register your Child
The following is the information you must provide in order to register a child:

Proof of Birth:  (One of the following must be provided)

  • Original birth certificate

  • Original baptismal certificate with raised seal

  • Current Passport or VISA
     

Proof of Immunization:

  • Statement signed by a physician or clinic listing the month, day and year of immunization, including: 2MMR (measles/mumps/rubella), 3DPT (diphtheria) and 3 polio vaccinations and one (Hib) Haemophilus Influenza
     

  • Three doses of Hepatitis B vaccination

  • One dose of Varicella vaccine

  • Documentation of Lead Screening for children under six years of age

Physical Exam by a Doctor is required for every new entrant.

Proof of Residency: (Three of the following must be provided):

  • Most recent Orange and Rockland utility bill and telephone or water bill. (Your name and address must be on bill)

  • Mortgage papers or tax bills in name of Parent or Legal Guardian

  • Original Signed Lease agreement accompanied by the following:

    Notarized paid rent receipt signed from the landlord with name and address of tenant and telephone number of landlord for verification purposes.

    Notarized letter of residency from property owner with their proof of residency.

Social Security Number

Parent entitlement affidavit form filled out and notarized
 

Affidavit of landlord or district resident form filled out and notarized (if applicable)
 

Last School Address:  (Including pre-schools)

  • The full address – street number, town, state, zip and telephone number.

Report Card: 

  • Please bring your child’s most recent report card with you to registration.  Since the transmission of records from the sending district can be delayed, it is of great help for the school to have a report card and high school transcripts, if available.
  • If your child receives special services, please bring his/her latest Individual Educational Plan (I.E.P.).

Custody Papers:

  • Original custody papers are necessary in cases of divorce or re-marriage. 
  • If you are not the parent of a child you wish to register, we will require valid custody papers with all necessary information.
